The passenger in the minivan: Edith Sharp, 58, of Forest Park.
Police said Mr. Moses radioed the Cincinnati/Northern Kentucky International Airport at 11:02 p.m. Saturday and said he was out of gas. They speculated he tried to land on the highway, but he was flying east and landed in the westbound lane. The plane and van crashed head-on just west of the Plainfield Road exit.
A witness, who did not want to be identified, said it appeared as though "the plane impaled the van."
The witness, a registered nurse, said she had just left a friend's house on Plainfield and was westbound on the highway when she saw cars begin to stop.
She said she got out of her car and saw the accident.
"I'm a nurse and I've seen things," she said, "but nothing compared to this. It was horrible."
The highway was reopened this morning. It was closed overnight while the bodies and wreckage were removed.
The crash occured just 15 hours after a Wyoming doctor died when his small plane crashed in a separate accident in Clermont County Saturday morning.
